What is St. John's wort?
St. John's wort, botanically Hypericum perforatum, is a native plant with bright yellow flowers that has been used traditionally since antiquity. Its key marker compounds are hypericin, a red pigment, and hyperforin, a light-sensitive bitter compound. Why St. John's wort causes so many interactions
The reason comes down almost entirely to a single compound: hyperforin. It activates a receptor called PXR in the liver and gut. Think of this receptor as a switch: once it's active, your body produces more of certain enzymes – above all CYP3A4 – and more of a transport protein called P-glycoprotein. Both are key tools your body uses to clear foreign substances.
The problem: a very large share of prescription drugs are broken down or transported out of the body via CYP3A4 and P-glycoprotein. Taking St. John's wort ramps up these clearance pathways – other medications get cleared from your body faster, before they've had their full effect. Blood levels drop, and so does how reliably the medication works.
This doesn't happen instantly: according to the European Medicines Agency (EMA), the increased enzyme activity builds up over several days of regular use – and takes about a week after stopping St. John's wort to return to normal. That matters in both directions: if a medication's blood level had been suppressed and you then stop St. John's wort, that level can rise again over the following week – with blood thinners, for example, one reason stopping should also be medically supervised. A second, independent mechanism affects the skin: hypericin accumulates there and reacts with UV light – the cause of the photosensitivity covered in the next section.
Side effects: what St. John's wort can cause on its own
Independent of other medications, St. John's wort can also cause side effects on its own, usually mild:
- Digestive complaints: nausea, abdominal pain, diarrhoea.
- Allergic skin reactions: rash or itching, even without sun exposure.
- Fatigue or restlessness: both are documented – and sound contradictory at first, which really just means: pay attention to how you personally react, not to a general expectation.
- Photosensitivity: by far the best-documented side effect. Fair-skinned people in particular become more sensitive to sunlight after taking St. John's wort – skin can react with tingling, redness, or sunburn-like reactions that would normally only appear after much longer, more intense sun exposure. A tanning bed has the same effect as real sun.
The risk rises with the dose: in trials using very high extract amounts over several weeks, skin sensitivity to UV rose markedly, while it barely showed up at typical intake levels. Still, the risk can't be ruled out entirely – so while taking it: avoid intense midday sun, use sun protection, and pause use if you notice unusual skin reactions.
Who this matters most for: medications that interact with St. John's wort

St. John's wort is one of the herbal substances with the most clinically documented interactions there is, recorded by bodies including the EMA and Germany's Federal Institute for Drugs and Medical Devices (BfArM). Affected groups include, above all:
- Hormonal contraceptives (the pill): hormone blood levels can drop. Documented effects include breakthrough bleeding and, in individual cases, unplanned pregnancies. Extra contraception or prior medical clearance isn't optional here.
- Coumarin-type blood thinners (warfarin, phenprocoumon): the anticoagulant effect can weaken, raising clot risk – and can swing the other way once you stop St. John's wort. Anyone on coumarins needs close monitoring of clotting values and should generally avoid St. John's wort.
- Immunosuppressants after organ transplants (ciclosporin, tacrolimus, sirolimus): the evidence here is especially clear-cut – documented cases exist where blood levels dropped far enough that donor organs were rejected. This combination is considered clearly contraindicated.
- HIV medications (such as protease inhibitors like indinavir, or NNRTIs like nevirapine): reduced effectiveness with a risk of resistance.
- Cancer medications (such as irinotecan, imatinib, tamoxifen): lower drug concentrations during a therapy where every deviation counts.
- Other substances with documented interactions: digoxin (heart), theophylline (airways), midazolam (sedation and anaesthesia), simvastatin and other statins, verapamil.
Antidepressants are a case of their own – a different mechanism is at play. It's not about faster breakdown, but about an amplified effect: combine St. John's wort with an SSRI such as paroxetine or sertraline, with trazodone, or with tricyclic antidepressants, and serotonin can build up too much in the brain. In rare cases this leads to serotonin syndrome, with sweating, a racing pulse, tremor, agitation, or confusion – a medical emergency that needs treatment.
This list isn't complete. If you take any medication regularly, one rule applies without exception: ask your pharmacist or doctor before your first St. John's wort capsule, not after.

- Extract ratio (DER): shows how concentrated the extract is, for example 10:1.
- Lab testing for purity, heavy metals, and residues – not a nice-to-have for a field-grown crop.
- Complete warnings in the product information or leaflet: a supplier that leaves out interactions is leaving out real safety information.
