Whether as drops, a spray or a lozenge: sublingual forms are popular among food supplements, above all for vitamin B12. The promise behind them sounds plausible: the active ingredient is meant to be absorbed directly through the oral mucosa, bypassing the detour through the digestive tract. But what's actually behind this technically, and does the evidence hold up to what advertising often suggests? This guide explains how sublingual absorption works and what really matters with B12 drops and sprays.
What does "sublingual" actually mean?
The term comes from Latin: "sub lingua" means "under the tongue". It refers to a way of taking a product where a liquid, or something that dissolves quickly, is placed under the tongue and held there for a few seconds to a few minutes before swallowing. The idea: the well-supplied mucous membrane on the floor of the mouth absorbs part of the active ingredient directly, so it doesn't have to pass through the stomach and small intestine first.
In pharmacology, sublingual administration is established for certain drugs, for instance where a rapid onset of action is needed or where an active ingredient would otherwise be broken down heavily in the gastrointestinal tract. This principle is readily adopted for food supplements too, though the evidence base here is considerably thinner than for approved medications. One important point for context: food supplements are foods, not medicines, and serve to supplement a normal diet.
How absorption under the tongue works
The oral mucosa is thin and threaded with fine blood vessels. Small molecules that are readily water- or fat-soluble can, in theory, cross this barrier and pass directly into the bloodstream. The advantage often cited: you bypass the so-called first-pass effect, meaning the initial breakdown of a substance in the liver and gut.
In practice, things are more complex. Contact time under the tongue is short, the absorbing surface is comparatively small, and a large part of the liquid gets swallowed anyway and is then processed in the gut. How much is really absorbed sublingually versus simply through the normal digestive route is hard to separate in everyday use. This is exactly where a critical look is warranted.
On top of that, the absorptive capacity of the mucosa varies from person to person and depends on factors such as saliva flow, molecule size and the exact formulation of the product. An active ingredient optimised for the sublingual route behaves differently from a simple solution placed under the tongue. That's why results from drug research can't simply be carried over to every food supplement. For consumers, this means: marketing claims about superior absorption should be read critically and weighed against the data that is actually documented.
Why vitamin B12 in particular is the focus
Vitamin B12 (cobalamin) is a special case. Its absorption in the body is normally tied to a complex mechanism: a protein produced in the stomach, intrinsic factor, is needed so that B12 can be absorbed in the final section of the small intestine. This route has a limited capacity. Alongside it, a second, passive absorption route exists, through which a small percentage is absorbed independently of intrinsic factor at high doses. This is precisely why high-dose B12 products make sense in the first place: even when the active transport route is saturated, the body still passively absorbs a bit more.
According to the authorised EU health claim, vitamin B12 contributes to "normal energy-yielding metabolism" and to "the normal function of the nervous system", as well as to "the reduction of tiredness and fatigue". These statements apply regardless of the form, so equally to drops, sprays, tablets and capsules.
What does the evidence say about sublingual B12 absorption?
Honesty is warranted here: the evidence is mixed. Several studies have compared sublingual B12 products with conventional oral tablets. The recurring finding is that both forms can raise blood B12 levels to a comparable extent. A clear, consistent superiority of the sublingual form over the swallowed tablet could not be demonstrated in these comparisons.
That doesn't mean drops or sprays are worse. It means that, given an adequate dose, they usually work just as well, but simply don't hold the special status they're often advertised with. Part of the effect of sublingual products is likely down to the swallowed portion and the passive absorption route anyway, not the mucosa alone. If you read that a spray is "many times better absorbed", treat such figures with caution, since robust evidence for them is usually lacking.
Drops, spray or tablet: where are the practical differences?
If pure absorption is often comparable, it is mainly practical criteria that determine the right form.
Drops
Liquid drops can be finely dosed and are well suited to people who don't want to swallow tablets. You place the stated number of drops under the tongue, hold them briefly, and then swallow. Pay attention to the B12 form and to whether the product contains alcohol, sugar alcohols or flavourings. The taste takes some getting used to, but dosing is flexible in return.
Sprays
Sprays are especially practical when out and about: one or two sprays into the mouth, and you're done. They deliver a fixed dose per spray and can be used discreetly. The downside compared with drops is less fine-grained dosing. Here too, it's worth checking the ingredients list, for example for sweeteners or carrier substances.
Tablets and capsules
Classic lozenges or swallowed tablets, as well as capsules, are often cheaper, keep for a long time and are easy to measure out. High-dose tablets make use of the same passive absorption route and are therefore a solid, often underrated option. It's not just the form that counts, but also what else is in the product.
The B12 form
Common forms are methylcobalamin, adenosylcobalamin, hydroxocobalamin and cyanocobalamin. Cyanocobalamin is well studied and stable, while the "bioactive" forms such as methylcobalamin are often advertised as more natural. In practice: all forms can cover B12 status if the dose is right. Sugar and additives
Sprays and drops in particular sometimes contain sweeteners, flavourings, alcohol or sugar alcohols such as xylitol. At the usual small serving sizes, the sugar content is generally negligible, but it's still worth taking an honest look at the label, for instance if you have intolerances or want to avoid sugar alcohols. A product labelled "no added sugar" isn't automatically free of sweeteners.
Using it correctly: timing and combination
For a sublingual B12 product to do its job, a calm, regular routine helps: place the drops or spray under the tongue, let them sit briefly, and don't immediately "wash them down" with food or drink. Who do B12 drops and sprays make sense for?
An increased requirement or lower intake particularly affects people who eat a vegan or predominantly plant-based diet, since B12 occurs almost exclusively in animal foods. Absorption can also become limited with increasing age or with certain gastrointestinal conditions. In these cases, supplementation can usefully support a normal diet.
Important: an actual deficiency, or symptoms you attribute to B12, should be clarified by a doctor. A blood test shows your status and whether supplementation is even necessary. Food supplements replace neither a balanced diet nor a medical diagnosis or treatment.
